In this package, you will find a list of our essential beauty services. Hair and Makeup services will ensure that you are a radiant, tranquil bride.

Also included are intake forms : these will be filled out before your intial complimentary 30 min. consultation.Changes may be made but must be 100% finalized 1 month prior to you scheduled wedding date

Lastly, you will find our contract agreement. Please carefully review, complete, and submit contract with a non-refundable 30% deposit

CONTRACT TERMS

BOOKINGS

Your first appointment will be a complimentary 30 min. consultation. Bride must come to this appointment with completed intake forms. During this appointment we will go over intake forms along with any questions and/or concerns you may have.we will then calculate total services needed to give us our estimated over all total. 30% of estimated total is due BEFORE scheduling your wedding day.

The remaining balance is due the day of the wedding.

Any service changes must be made 1 month prior to scheduled wedding date (see "change order" form)

ADDITIONAL FEES

Hair must be clean and dry on the day event. Skin must be clean and moisturized prior to coming in for the day of event or there will be additional fees applied.

Additional fees,blow out (on already wet hair) ($30), makeup removel ($10), strip lash removel ($10)

Any additional services made day of event must be paid in full on day of event

CHANGE ORDERS

Service changes must be made.in writing, 30 days prior to the scheduled wedding date. Please complete and submit a "change order" form in order to change estimates total. Estimated total will be locked in 30 days prior to scheduled wedding date. Services listed but not completed day of will still be included in estimated total. Any additional services will be additional to estimated total. Failure to pay for any additional services will result in a charge to the credit card listed below

LIABILITIES

I, understand and agree to give stylists/artists knowledge of any and all known bride and attendees "bridal party" (that are receiving services) allergy ingredients that could potentially be found in beauty products. These allergies are to be listed in the "Allergies" section. The bride and/or attendees (receiving services) understand and agree that if notices is NOT given to artists/stylist of any known allergies that brides and/or attendees, or if bride/attendees are not aware they have an allergy. artist/stylist cannot be held liable for any reactions.

All tools and products are kept sanitary. All products are sanitized between every makeup application. Any skin conditions should be reported by the client to the artist/stylist prior to application. artist/stylist cannot be held liable for any reactions.

CANCELLATION POLICIES

Cancellations may be made up yo 60 days before scheduled wedding date without extra charges. Your original deposit of 30% is non refundable.if you cancel anytime between 59 to 30 days before your scheduled wedding date, the remaining 20% (50%) of total estimated rate will be charges. if you cancel anytime between days to day of your scheduled wedding date 100% of your total estimated rate will be charged. Cancellations must be submitted to Hait Gallery at the Mill in writing. Your final balance (if any) will then be due 2 weeks after the written cancelation is approved. Must include credit card information in order to schedule your wedding party, card will NOT be charged unless payment is not made

FINAL PAYMENT

Final payment is due on the of event as one payment when makeup artist/hair stylist has completed all scheduled and additional services. The person responsible for the final payment is the person who has signed the contract. Payment must be made in either cash or credit card.

Any day of additional services will be calculated and added below along with client signarure.
